What is it used for?
An Unfair Labor Practice charge may be filed by a public employee against a public employer or an employee organization. An Unfair Labor Practice charge may also be filed by an employee organization against a public employer and vice versa. The entity or individual who files the charge is referred to as the Charging Party. The entity against whom the charge is filed is referred to as the Charged Party.

A public employee, employee organization or employer having reason to believe that an unfair labor practice may have been committed may use this form.
