What is it used for? 

The Notice of Appearance ensures that all communication regarding a case is directed to each party’s designated representative.  

Who uses this form?  

A Notice of Appearance is filed in each new or pending case by any person who will be the representative of a party in that case, including the parties who may represent themselves. If the representative changes during the processing of a case, a new Notice of Appearance must be filed.
