Employee Organization Registration Report Form

What is this form used for? 

This form is used to register a new employee organization with SERB. This form must be filed within sixty days after certification or recognition of the bargaining unit. This form is only filed once for each new union.  

Failure to comply may result in revocation of bargaining unit certification.
