Employee Organization Annual Report Form

What is this form used for? 

This form is used to file the required Employee Organization Annual Report. A completed 12-month financial statement must also be attached to this filing.  

Failure to file this form on time could result in revocation of the bargaining unit certification.  To avoid this penalty, SERB sends three reminder notifications near the due date to the employee organization.

Who uses the form? 

A representative of the employee organization files this form annually within 5 months and 15 days after the fiscal year end.
